Empire offers three levels of NEMT service to customers in Gwinnett County and select counties in North Georgia:

Arrive on time
1) Request
Send us a request providing details for your transportation needs — level of service, addresses, pickup time and appointment time.
2) CONFIRM
We will notify you once your trip is confirmed and send the amount due. Once payment is submitted, your trip is considered scheduled.
3) RIDE
Your assigned driver will communicate with you before and upon arrival. If needed, mobility assistance will be provided to/from the vehicle.
